Log message:
	Add method linear_system_insert, a slightly modified copy of
	Linear_System::insert.  Update insert to use linear_system_insert
	instead of trying to handle parameters specially.  Make ascii_dump() a
	real (vs inline) method.  Set the affine_image grid parameter
	automatic value to true.
