Log message:
	Add empty_reduce_d1_with_d2() and empty_reduce_d2_with_d1().  Define
	add_generator().  Add Grid Polyhedron specialisations off
	add_generator() and of the grid generator constructors.  Take out grid
	methods intersection_assign_and_minimize, join_assign_and_minimize and
	join_assign_if_exact.
