Log message:
	The function term_to_polyhedron_handle() replaces get_ph_pointer().
	The new function either return a plausible polyhedron handle or
	throws the `not_a_polyhedron_handle' exception.
	All sites that used to call get_ph_pointer() have been consequently
	simplified.
